• Do not initiate new members prior to receiving the sealed white copies of Form 2 from the National Secretary.
• These forms, embossed with the seal of Chi Epsilon, are your authority to initiate. If your order is received on time, the white copies will be returned long before the initiation.
• INITIATION WITHOUT AUTHORIZATION CALLS FOR A SEVERE PENALTY; for the 1st offense, a $10.00 fine (see Bylaws, p. 29), for subsequent offenses a $50.00 fine (see Bylaws, pg. 29), or worse, possible "Probation" for your chapter.
• Notify the National Office by letter, FAX, or e-mail of the name and phone number of your new Faculty Advisor, if a change has been made recently or between Semi-Annual Reports. Please remember that your Faculty Advisor MUST be a member of Chi Epsilon.
• If some of your forms have been lost or misplaced, contact the National Office immediately for replacements (at a nominal cost).
• Please discard old out-of-date Form 1s. Using old forms just delays your order. If you are out of forms, please call us for more – or download from our website at www.chi-epsilon.org.
