• One Legacy Official Order Form, Form 1 (Page 15)---NEW, note price changes!
• One Legacy Key Engraving Instructions, Form 1-B (Page 17)
• One Legacy Membership Certificate Order, Form 1-A (Page 16) (carbonless)
• Several Form 2's, (Page 18) [Permanent Membership Record] (one per initiate)(carbon paper)
• One Form 3 (Page 19) -- Scholarship Certification (carbonless).
• One Form 4 (Page 20)-- Semi-Annual Report (carbonless).
