1. CHAPTER Order Forms to be sent to
National Office
a) FORM 1 – Official Order Form
(example on pg 15)

When filling
in the shipping information (upper left
corner on Legacy Order Form 1), you are required to use
your Faculty Adviser's name, school and address to have
Legacy send the completed keys and certificates to your
Advisor. Do not use your personal address.
b) FORM 1a – Legacy, Membership
Certificate Order (example on pg 16)
c) FORM 1b – Legacy, Key Engraving Instructions
(example on pg 17)
d) FORM 2 – Permanent Membership Record
(example on pg 18)
e) FORM 3 – Scholarship Certification
(example on pg 19)

Use the
candidate’s full name (see item c)

All columns
must be filled out for each undergrad
candidate – No exceptions

For Grads,
rank and # in class info not needed, just put “yes”
in the Grad column

CE Dept.
Head, verifying that the students listed are in the upper
third of their class, must sign. Rank
of each student is preferred and requested.

Faculty
Advisor must sign at bottom where indicated.
f) A CHECK, PURCHASE ORDER, UNIVERSITY VOUCHER,
OR CREDIT CARD INFORMATION (on Form 1) MUST BE SENT WITH
CHAPTER ORDER.

Please do not send multiple
checks from members, send only one check for the
full amount.

Make your check payable to:
Chi Epsilon, and include it with your order.
Please increase your payment by your chapter's outstanding balance, if there is any.

2. FORMS SENT TO National
Office AFTER INITIATION
a) Form 4 – Semi-Annual Report (Page
20)

Must be completed with
the names, email addresses and phone #’s of each
officer each semester without delay (and when any changes
occur).

Must be received within 10 days after
election/installation of new officers to avoid a $10.00
late fee (see Bylaws, p. 29).
b) Form 5 - Certificate of Initiation
(white card). (Page 21)

Is to be completed and mailed to this
office without delay after the initiation of new members.

The Constitution sets a deadline for
this – ten (10) days after the initiation,
to avoid a $10.00 late fee (see Bylaws, p. 29).

The form must be typewritten or lettered
in black ink (not pencil) – Permanent Record.
c) Form 7 - Certificate of Non-Initiation
(red card). (Page 21)
- Use if a candidate paid fees, was listed on order, but
did not actually go through the initiation ceremony.
- Candidate can still go through initiation later.
3. Forms National Office
sends to each chapter after an order has been processed
a) Package 1 (Sent First
Class Mail)

Chapter’s
copy of order Forms 1, 1a, 1b, Form 2’s (sealed
white copy), Form 5’s (one for each initiate), and
Form 7’s.
b) Package 2 – (Sent
Fourth Class Mail)

New set of Order Forms --- One each
of 1, 1a, 1b, 3, & 4

Same # of Form 2’s that were to
be initiated

Enough copies of the Constitution &
Bylaws, Newsletter, and Transit for each new initiate.
